1. Acceptance of Terms
By using Mycelium Pharmacy services, you agree to these Terms of Service and our
Privacy Policy.
2. Service Description
Mycelium Pharmacy provides telemedicine consultations with our licensed physicians,
prescription management, and direct medication fulfillment. Our technology platform,
billing, and customer support operations are managed by Serena Health.
3. Eligibility
- You must be at least 18 years old
- You must be located in a state where our physicians are licensed
- You must provide accurate and complete information
4. Medical Disclaimer
- Mycelium Pharmacy is NOT a replacement for emergency medical care
- Our physicians provide telemedicine evaluations for specific treatment categories
- Prescriptions are at the sole discretion of the evaluating physician
- A physician may decline to prescribe for any medical reason
5. Payment Terms
- Prices are displayed on our website and may change
- Payments are processed by Serena Health on behalf of Mycelium Pharmacy through Stripe
- Charges may appear on your statement as "MYCELIUM*SERENA" or "SERENA HEALTH"
- Subscription plans auto-renew unless cancelled
- Refund requests: Contact support within 14 days for unused medications
6. Prescription & Fulfillment
- Prescriptions are written by Mycelium Pharmacy's licensed physicians
- Medications are prepared and shipped directly by Mycelium Pharmacy
- Shipping times are estimates, not guarantees
- You are responsible for providing accurate shipping information
7. Customer Support
Customer support is provided by Serena Health on behalf of Mycelium Pharmacy.

8. Limitation of Liability
Mycelium Pharmacy and Serena Health are not liable for adverse medical reactions,
shipping delays beyond our control, or damages arising from inaccurate information you provide.

We may update these terms. Continued use after changes constitutes acceptance.
10. Governing Law
These terms are governed by the laws of the State of Florida.
