Mycelium Pharmacy ("Mycelium," "we," "us") is a telemedicine and prescription pharmacy service. We employ our own network of licensed physicians who conduct medical evaluations and write prescriptions. We handle prescription fulfillment and ship medications directly to patients.

Our technology platform and billing operations are managed by our partner, Serena Health ("Serena"). Serena Health handles payment processing, customer support, and platform infrastructure on behalf of Mycelium Pharmacy.

2. Information We Collect

Personal information: name, email, phone number, shipping address
Payment information: processed securely through Stripe by Serena Health (we do not store card numbers)
Medical information: collected through secure intake forms for physician review
Usage data: how you interact with our website

3. How We Use Your Information

To provide telemedicine evaluations by Mycelium Pharmacy physicians
To process prescriptions and ship medications
To process payments through Serena Health's billing system
To communicate about your treatment (emails, SMS)
To provide customer support (managed by Serena Health)

5. HIPAA Compliance

Mycelium Pharmacy is committed to protecting your health information in accordance with HIPAA and applicable state privacy laws. A Business Associate Agreement governs the handling of Protected Health Information between Mycelium Pharmacy and Serena Health.

7. SMS Communications

By providing your phone number, you consent to receiving transactional SMS messages about your treatment. You may opt out at any time by replying STOP.

8. Bank Statement Appearance

Because billing is processed by Serena Health on behalf of Mycelium Pharmacy, charges on your bank or credit card statement may appear as "MYCELIUM*SERENA", "SERENA HEALTH", or "MYCELIUM PHARMACY".

9. Data Security

We use industry-standard encryption, secure hosting, and access controls to protect your information.
